Do you accept insurance?

I’m a cash-based practice and do not bill insurance directly. However, many clients pay with HSA/FSA funds or submit superbills for out-of-network reimbursement. I can guide you through that process. I do not accept Medicare, and because I travel to clients’ homes and dedicate a full hour (or more) per session, I’m not enrolled in any insurance networks. If your policy includes out-of-network benefits, I’ll provide a detailed invoice (superbill) you can submit for potential reimbursement upon request. Payment is required at the time of service.

What are your rates?

For rates and scheduling, please call or text 614-300-7924, or use the contact form below. I’m happy to walk you through the options and answer any questions.
Initial Evaluation (75 minutes)
Follow-Up Visits (60 minutes)
Payment is due at the time of service.
Package options are available—please ask for details during your consultation. HSA/FSA cards are accepted, and superbills are provided for out-of-network reimbursement upon request.

Why choose a cash-based model?

Choosing a cash-based model allows me to offer truly one-on-one, 60-minute sessions without any interruptions. You get:
Consistent care from me (no rotating clinicians)
Flexible, convenient scheduling (no insurance approvals needed)
Direct access to me via email, phone, or text between sessions.
This focused approach often leads to faster results and fewer total visits, which can save you money over time—despite being out of network.

What should I expect during a visit?

Each visit is one-on-one with me, Dr. Derek Speaker, DPT, at your home, office, garage, or even an outdoor space—whatever works best for you. Sessions are tailored to your goals, using evidence-based rehab and performance strategies.

What equipment do you bring?

I come fully equipped with:
A portable treatment table.
Basic medical tools (blood pressure cuff, pulse oximeter, etc.)
Resistance bands and other therapy supplies as needed.

What happens during my first home PT visit?

Your first session includes:
A thorough evaluation and movement assessment.
Hands-on treatment tailored to your needs.
A custom home exercise plan.
Time for any questions you have.
The session lasts about 75 minutes, and I bring all necessary equipment.

Do I need a doctor’s referral?

No referral is required in Ohio. You can see a physical therapist directly. If your condition requires medical clearance, I’ll help coordinate with your physician.

Who do you treat?

I work with:
Active adults and athletes.
Seniors needing balance or fall-prevention support.
Post-surgical patients.
Anyone looking to reduce pain, improve movement, or prevent injury.
Whether you’re recovering from surgery or trying to stay mobile as you age, I tailor care to your unique needs.

How do I get started?
